In the Health and Safety Event form it is possible for users to add new items to the event cause, agency and contributing factor dropdown lists, if the appropriate permissions are enabled. This means that as users are completing the event form, they are able to add new items to the list if the appropriate item isn't available.

When causes/agencies/contributing factors are added to the list by a user, they will appear in the list with (Pending Approval) next to the item. This indicates to an admin user that can approve the new item in the list or re-map it to an existing cause/agency/contributing factor.


The new item appears in the list with a 'Pending Approval' tag.

To review and accept/delete new items, go to System Settings > Health Safety Form Settings > Actions Causing Event (or Objects/Condition Cause of Event, Contributing Factors).

The active causes are on the left and the inactive (or 'pending approval' causes are on the right.

To delete and re-map a cause, click the Delete button next to the cause.

You will then be asked to re-map the cause to an active cause. Select an appropriate cause to re-map to.

You may want to refer to all events that have had this cause selected, to ensure that you are re-mapping to the appropriate cause. The easiest way to do this is to download a full csv file of all events, which is available in System Settings > CSV Integration > 11. HSE Incidents.

When you click 'Map', the item will disappear from the left of inactive causes on the right. That item will no longer appear in the HSE event form, in the Action Causing Events dropdown list.

Any health and safety events that previously had this cause attached to them will have been remapped to the active cause that you selected.
